Today the city manager released an internal investigation report after an officer shot another officer in April.

Lynchburg City Manager Kimball Payne released an internal report this morning describing what happened moments before and after an officer was shot in April by another officer.

The report comes after an internal investigation from the Lynchburg Police Department detailing what went wrong when they say Officer Gary Hilber was shot by another officer, Taylor Clark, outside the McCausland Ridge Apartments on Langhorne Road.

In a three page report, Police Chief Parks Snead says the officers didn't communicate properly with each other before the shooting happened.

The report states not everyone in the Street Crimes Unit knew Officer Hilber was at the scene in plain clothes working with another unit.

Snead goes on to say this: "We have learned from our mistakes, we are actively addressing these mistakes, and we will do everything within our power to ensure that mistakes like these do not recur."

Tune in tonight on WDBJ7 for more on the shooting investigation.
